Clinical and Roentgenographic Observation on Hypertrophic Pulmonary Osteoarthropathy


Abstract
In two patients receiving treatment under the clinical diagnosis of rheumatoid arthritis, a diagnosis of hypertrophic osteoarthropathy was made by X-ray studies of the bones and joints and primary pulmonary lesions were also found.
Periosteal reaction, principally in the long bones, overlying soft tissue swelling and pain in the ankle and wrist joints appeared earlier and was worse on the side of the primary pulmonary lesion.
In case 1, 50 years old Korean mile, soft tissue swelling and pain in the wrists and ankles dramatically diminished within two days of operative removal of the pulmonary lesion, confirmed as adenocarcinoma of the lung, although it has taken seven weeks for the above mentioned signs and symptoms including periosteal reaction to disappear completely. However, it was found that soft tissue swelling and pain lasted longer post-operatively in the extremities on the same side as the primary pulmonary lesion.
Hypertrophic periosteal reaction on the lateral margins of the scapulae, as in case 2, 50 years old Korean male, is a rare occurence.
In cases where the results of treatments are not satisfactory in patients with a clinical diagnosis of rheumatoid arthritis or other arthridides involving multiple joints, X-ray studies of bones, joints and chest should be obtained.
